Papież Urban VI poleca trzem wymienionym egzekutorom (biskupowi lubuskiemu, prepozytowi krakowskiemu oraz kantorowi wrocławskiemu), by mocą władzy apostolskiej udzielili prowizji Janowi Raczkowicowi de Schierusko, familiarisowi cesarzowej Elżbiety, na kanonię kolegiaty św. Idziego za murami Wrocławia (15 grz.), wakującą w Stolicy Apostolskiej po tym, jak papież pozbawił wszystkich beneficjów Teodoryka z Głogowa/Głogówka, pomimo że Jan niedawno otrzymał papieską prowizję na kanonię wrocławską.
